P DualChoice (HMO D-SNP) is a Health Plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. It is an organization made up of doctors, hospitals, pharmacies, providers of long-term services and supports, behavioral health providers, and other providers. It also has care coordinators and care teams to help you manage all your providers and services. They all work together to provide the care you need.   What is the goal of this program? s for Contracted and Non-Contracted Providers Emergency and Post-Stabilization Care for IEHP Members IEHP DualChoice (HMO D-SNP) Model of Care Training for Non-Contracted Providers Provider Referral Outside of the IEHP Network Provider Dispute Resolution Process for Contracted and Non-Contracted Providers   Definition of a Provider Dispute A provider dispute is a written notice from the provider to Inland Empire Health Plan (IEHP) that: Challenges, appeals, or appeals, or requests reconsideration of a claim (including a bundled group of similar claims) that has been denied, adjusted, or contested Challenges a request for reimbursement for an overpayment of a claim Seeks resolution of a billing determination or other contractual dispute   What is not Considered to be a Provider Dispute Claims denied for missing or additional documentation requirements such as consent forms, invoices, Explanation of Benefits from primary carrier, or itemized bills are not considered Provider Disputes Corrected Claims Pre-Service Authorization Denials   Provider Dispute Time Frame IEHP accepts disputes from providers if they are submitted within 365 days of receipt of IEHPs decision (for example, IEHPs Remittance Advice (RA) indicating a claim was denied or adjusted).   Submission of Provider Disputes When submitting a provider dispute, a provider should use a Provider Dispute Resolution Request form. If the dispute is for multiple, substantially similar claims, complete the spreadsheet on page 2 of the Provider Dispute Resolution Request Form All Provider Disputes and supporting information must be submitted to: IEHP Claims Appeal Resolution Unit PO BOX 4319 Rancho Cucamonga, CA 91729-4349   Acknowledgement of Provider Dispute IEHP acknowledges receipt of each provider dispute, regardless of whether the dispute is complete, within 15 business days of receipt.   Resolution Timeframe IEHP resolves each provider dispute within 45 business days following receipt of the dispute, and provides the provider with a written determination stating the reasons for determination.   PDR Determination Resulting in Additional Payment If IEHP determines to pay additional monies based on information originally provided and/or available at the time the claim was first presented to IEHP for adjudication, or a result of a processing error IEHP will automatically include the appropriate interest amount if payment is not issued within required regulatory timeframes.   Non-Contracted Provider Disputes Resolution Process for IEHP DualChoice (HMO D-SNP) A non-contracted provider, on his or her own behalf, is permitted to file a standard appeal for a denied claim only if the non-contracted provider completes a waiver of liability statement, which provides that the non-contracted provider will not bill IEHP DualChoice (HMO D-SNP) Plan Members.   Who to Call with Questions on IEHPs PDR Process Contracted providers may visit our online secure provider portal at www.iehp.org for more information. Providers may also call the IEHP Provider Call Center at (909) 890-2054 or (866) 223-4347 Monday-Friday, 8:00 am to 5pm PST.   (Back to Non-Contracted Providers Menu) Emergency and Post-Stabilization Care for IEHP Members   Triage and Advice Systems IEHP provides Members triage, screening, and advice services by telephone 24 hours a day, 7 days a week through its Nurse Advice Line (NAL). By calling the NAL, Members receive assistance with access to urgent or emergency services from an on-call physician, or licensed triage personnel. IEHP Members can reach this 24/7 Nurse Advice Line at (888)-244-IEHP (4347) or 711 (TTY).   Post-Stabilization Care IEHP requires contracted and non-contracted hospitals to obtain prior authorization for post-stabilization care for Members (patients). IEHP requests the patient’s diagnosis as indicated by the treating physician or surgeon and any other information reasonably necessary for the Plan to decide on whether to authorize post-stabilization care or to assume management of the patient’s care by prompt transfer to another facility. The hospital should request prior authorization from IEHP’s Utilization Management (UM) Department by: Phone at (866) 649-6327; or Fax at (909) 477-8553 to send clinical notes for medical necessity review.   IEHP makes every effort to respond to requests for necessary post-stabilization care within thirty (30) minutes of receipt. The services are considered approved if IEHP does not respond within this timeframe. All subsequent hospital day are subject to review for medical necessity. IEHP will inform the provider of the Plan’s decision and will coordinate the transfer of the Member if IEHP denies the request for authorization of post-stabilization care and elects to transfer the Member to another health care provider.   Non-Emergency Services If a Member presents at the emergency department for non-emergency services, please refer the Member to their IEHP Member Handbook, section 3 (How to Get Care), which outlines the process for obtaining a referral.   Claims Reimbursement Complete facility claims for authorized health care services must be sent to: Inland Empire Health Plan Attn: Claims Department – IEHP Direct PO BOX 4349 Rancho Cucamonga, CA 91729-4349 Complete professional claims for authorized health care services must be sent to: For IEHP-Direct Members, please send to address above. The Member’s IPA will review the request for referral and provide a decision within regulatory timeframes. The Member’s IPA will approve the request if it is deemed medically necessary and if IEHP or the IPA does not have an appropriate alternative provider available within its network. 2-2023 influenza season continues to coincide with circulation of COVID-19 virus (SARS-CoV-2). As of August 2022, approximately 94.2 million cases of COVID-19 had been reported in the United States. Influenza vaccination remains an important tool for the prevention of potentially severe respiratory illness, which helps decrease the stress on the U.S. health care system. IEHP DualChoice members who are 18 years of age or older may obtain flu vaccines through the IEHP Pharmacy Vaccine Network. Vaccine Notice: Access to Pharmacy Vaccine Network (PDF) Frequently Asked Influenza (Flu) Questions: 2022-2023 Season (CDC Recommendations) What’s New for 2022-2023  By clicking on these links, you will be leaving the IEHP website. The composition of flu vaccines has been updated. For the 2022-2023 flu season, there are three flu vaccines that are preferentially recommended for people 65 years and older. These are Fluzone High-Dose Quadrivalent vaccine, Flublok Quadrivalent recombinant flu vaccine and Fluad Quadrivalent adjuvanted flu vaccine. The recommended timing of vaccination is similar to last season. For most people who need only one dose for the season, September and October are generally good times to get vaccinated. Vaccination in July and August is not recommended for most adults but can be considered for some groups. While ideally it’s recommended to get vaccinated by the end of October, it’s important to know that vaccination after October can still provide protection during the peak of flu season. The age indication for the cell culture-based inactivated flu vaccine, Flucelvax Quadrivalent (ccIIV4), changed from 2 years and older to 6 months and older. Pre-filled Afluria Quadrivalent flu shots for children are not expected to be available this season. s the Medi-Cal Renewal process, is currently underway across our state. This initiative is the biggest challenge facing the Medi-Cal program in its history. Up to 400,000 IEHP Members could potentially lose their Medi-Cal coverage if they don't complete the necessary renewal paperwork on time. Puede obtener servicios como los mencionados abajo sin obtener aprobación con antelación de su Proveedor de Atención Primaria (PCP). Atención médica de rutina para mujeres, incluyendo exámenes de senos, mamografías (radiografías de los senos) de detección, pruebas de Papanicolaou y exploraciones pélvicas siempre y cuando las obtenga de un proveedor de la red. Vacunas de la gripe, siempre y cuando las obtenga de un proveedor de la red. Servicios de emergencia de proveedores de la red o de proveedores fuera de la red. Atención de urgencia de proveedores de la red o de proveedores fuera de la red cuando los proveedores de la red estén temporalmente no disponibles o inaccesibles, p. ej., cuando esté temporalmente fuera del área de servicio del plan. Servicios de diálisis renal que reciba en un centro de diálisis con certificación de Medicare cuando esté temporalmente fuera del área de servicio del plan. (Si es posible, llame a Servicios para Miembros de IEHP DualChoice antes de irse del área de servicio para que podamos ocuparnos por usted de organizar su diálisis de mantenimiento cuando esté lejos). Cómo obtener atención médica de especialistas y otros proveedores de la red Un especialista es un Doctor que presta servicios de atención médica por una enfermedad específica o para una parte del cuerpo. Normalmente, verá primero a su PCP para la mayoría de sus necesidades de atención médica de rutina, como chequeos físicos, inmunizaciones, etc. Cuando su PCP considere que usted necesita suministros o tratamientos especializados, su PCP tendrá que obtener autorización previa (es decir, aprobación con antelación) de su Plan o grupo médico. Esto se llama referencia. Su PCP enviará una referencia a su plan o grupo médico. Es muy importante que obtenga una referencia (aprobación con antelación) de su PCP antes de ver a un especialista del u otros proveedores determinados del Plan. Si no obtiene una referencia (aprobación con antelación) antes de recibir los servicios de un especialista, es posible que tenga que pagar usted mismo estos servicios. Los PCP suelen estar vinculados a determinados hospitales y especialistas. Al elegir un PCP, también se determina qué hospital y especialista puede usar.  ¿Qué pasa si un especialista u otro proveedor de la red se va de nuestro plan? A veces es posible que un especialista, una clínica, un hospital u otro proveedor de la red que usted esté usando se vaya del plan. Cuando un proveedor se vaya de una red, le enviaremos por correo una carta en la que lo informaremos de su nuevo proveedor. Si prefiere uno diferente, llame a Servicios para Miembros de IEHP DualChoice y lo asistiremos para que busque y elija otro proveedor. Cómo obtener atención médica de proveedores fuera de la red Cuando su doctor recomienda servicios que no están disponibles en nuestra red, puede recibirlos de un proveedor fuera de la red. Para recibir servicios fuera de la red, su Proveedor de Atención Primaria (PCP) o especialista debe presentar una solicitud de referencia a su plan o grupo médico. Procurement department is continuously looking for suppliers of the varied goods and services it procures.  IEHP procures goods and services through the solicitation process, and in the case of repetitively purchased items, establishes long-term contracts. With the exception of Public Works (construction type bids) and a few specialty bids, most bids for goods and services procured are completed using a third-party solicitation website called Bonfire. Vendors have the option to view IEHP’s open solicitations on the Bonfire website. IEHP invites all vendors to register with Bonfire and participate in IEHP’s fair and open solicitation process for goods and services. s hemos escuchado que comer bien y hacer actividad son los secretos de una vida larga y sana. Esto es importante. Pero hay más que usted puede hacer para detener las enfermedades. Encontrar problemas de salud precozmente le da más opciones de tratamiento o cura. Para eso son las evaluaciones médicas.  Las evaluaciones médicas son vitales para todas las personas, desde los recién nacidos hasta los adultos mayores. Estas pruebas están diseñadas para buscar señales de que usted podría estar en riesgo de tener ciertas condiciones. Ayudan a detectar problemas de salud de manera precoz, aunque usted no tenga síntomas.  Tipos de evaluaciones para el cáncer Cáncer de los senos Una mamografía es una radiografía de los senos. Puede detectar el cáncer de los senos de manera precoz, cuando es más fácil de tratar y cuando su probabilidad de cura es mucho más alta. Los hombres y las mujeres pueden tener cáncer de los senos, y deben hacerse evaluaciones cada dos años a partir de los 50 años. Las personas que tengan riesgo elevado o que quieran comenzar con las evaluaciones antes de esa edad pueden hablar con su Doctor de empezarlas a los 40 años. Cáncer de cuello uterino (examen de Papanicolaou)  La prueba de Papanicolaou, también conocida como pap, puede detectar células anormales en el cuello del útero tan precozmente, que se pueden tratar antes de que el cáncer tenga la posibilidad de crecer. La evaluación se recomienda para las mujeres de 21 a 65 años, cada 3 a 5 años, según el riesgo y el tipo de evaluación. Para programar una evaluación, hable con su Doctor.  Cáncer colorrectal   El cáncer colorrectal afecta al colon y al recto. Las evaluaciones pueden encontrar y eliminar excresencias en estas áreas antes de que se conviertan en cáncer. Todas las personas de 45 a 75 años deben hacerse evaluaciones de cáncer colorrectal cada 1-10 años, según el riesgo y el tipo de prueba usada. Hable con su Doctor sobre cuáles es la mejor prueba para usted.   Cáncer de pulmón Las evaluaciones de cáncer de pulmón con imágenes (CT) en las personas que fumen o que hayan dejado de fumar en los últimos 15 años pueden ayudar a encontrar el cáncer en sus comienzos. Detectarlo precozmente lo mantendrá sano. Se recomienda una evaluación anual para todas las personas de 50 a 80 años que fumen cigarrillos o que hayan dejado de fumar en los últimos 15 años.  Si tiene que hacerse una evaluación, llame a su Doctor hoy para programar su cita. Los Servicios de Apoyo Comunitario pueden ayudarlo a mantenerse sano, a disminuir las complicaciones por enfermedades y a evitar estancias innecesarias en el hospital, en centros de enfermería y en departamentos de emergencias Es posible que los Servicios de Apoyo Comunitario con IEHP lo ayuden durante una transición de vivienda o en la atención. Si necesita ayuda para manejar su salud, los Servicios de Apoyo Comunitario de IEHP podría ser la opción indicada. Si necesita más atención Los Servicios de Apoyo Comunitario de IEHP se ofrecen a para cualquier Miembro de Medi-Cal o de IEHP DualChoice Cal MediConnect Plan (plan de Medicare-Medicaid) que necesite atención de apoyo. Los Servicios de Apoyo Comunitario pueden favorecer la salud general, cuidando el cuerpo y la mente. Los Miembros que podrían beneficiarse de los Servicios de Apoyo Comunitario son aquellos que: Pueden estar enfrentando la falta de vivienda.
