This service offers help and support with the first days at home, return to work, support groups, and timely answers to challenges nursing mothers face. Members can directly contact the Loving Support 24/7 Helpline at 888-451-2499. No referral is necessary. English and Spanish-speaking certified lactation specialists and Internationally Board Certified Lactation Consultants (IBCLCs) are available 24 hours a day, 7 days a week to answer questions. Foundation Leadership Angelica Baltazar, MSW, serves as the IEHP Foundation President and has more than 20 years of experience in the public health and human services sectors. She holds a bachelor’s in social work from Cal Poly Pomona and a master’s in policy planning and administration from Loma Linda University. Baltazar is a board member of the Greater Ontario Business Council, Promise Scholars and Partners for Better Health. Daniels, quien ejerce en dos clínicas operadas por Woman to Woman OB/GYN Medical Group. “IEHP es uno de los mejores defensores de pacientes que hay”. Desde el compromiso de ayudar a los Proveedores a bajar los índices de cesáreas hasta el reciente lanzamiento de un programa de doula, la Dra. Acreditación del NCQA IEHP fue el primer plan de salud “sólo de Medicaid” en California en ganar la acreditación de National Committee for Quality Assurance (NCQA) para su plan de salud Medi-Cal en 2000. Desde ese año conservamos la acreditación NCQA.  Premios a la Innovación En 2018, IEHP recibió el Premio a la Innovación otorgado por el Departamento de Servicios de Atención Médica (Department of Health Care Services), este es un reconocimiento por promover el Acceso a la Vivienda, el cual ofrece apoyo a través de alojamiento permanente y asistencia con servicios intensivos para coordinar los casos de los Miembros sin hogar con un alto nivel de utilización de servicios. En octubre de 2017, IEHP ganó el Premio a la Innovación del Departamento de Servicios de Salud de California (Department of Health Care Services, DHCS) por mejorar la salud de los Miembros de bajos ingresos a través de un programa que coordina sus necesidades complejas de atención médica para la salud física y del comportamiento, por medio del sistema de atención médica en los Condados de Riverside y San Bernardino. En 2015, IEHP ganó el primer Premio a la Innovación anual de DHCS por su Programa de Asistencia de Transición, este programa se concentra en reducir el uso de la sala de emergencias y limita las tarifas de readmisión de los Miembros recién dados de alta del hospital. IEHP recibe el premio al Socio Comercial del Año del Distrito de Protección contra Incendios de Rancho Cucamonga En septiembre de 2021, el Distrito de Protección contra Incendios de Rancho Cucamonga otorgó a IEHP el premio al Socio Comercial del Año en la celebración del reconocimiento a la policía y los bomberos de la ciudad. El distinguido premio reconoce la asociación del plan de salud con la ciudad y el condado de San Bernardino para albergar una clínica continua de vacunación masiva en las oficinas centrales del plan de salud. IEHP DualChoice Cal MediConnect Plan Obtiene un Alto Reconocimiento en Satisfacción al Cliente a Nivel Nacional IEHP DualChoice Cal MediConnect obtuvo la tercera calificación más alta a nivel nacional en la encuesta CAHPS de MMP 2016 de los Centros de Servicios de Medicare y Medicaid (Centers for Medicare and Medicaid Services, CMS), los cuales evalúan las experiencias de los pacientes con sus respectivos planes de salud. En 2016 participaron 40 Planes de Medicare-Medicaid (Medicare-Medicaid Plans, MMP) en el país (llamados Cal MediConnect plans in California).  Premio Anual a la Excelencia En 2016, IEHP recibió el Premio a la Excelencia que otorga Pharmacy Benefit Management Institute (PBMI) por su Programa de Pago por Desempeño para Farmacias (Pharmacy Payment for Performance), el cual ofrece educación clínica a sus Miembros a través de las farmacias de la red de IEHP. Even if you are not sick now, planning for health care in the future is an important step toward making sure you get the medical care you would want, if you are unable to speak for yourself and doctors and family members are making the decisions for you. MY LIFE. MY CHOICE. is IEHP’s Advance Care Planning program for IEHP DualChoice Members. The online program helps you fill out Advance Care Directive forms and gives you a place to keep them safe to share with your Doctor. IEHP can also store related documents like Physician Orders for Life-Sustaining Treatment, Living Wills, Health Care Power of Attorney and Do Not Resuscitate forms. MY LIFE. MY CHOICE. is accessible through the IEHP Member Portal. You can easily upload your Advance Care Directives and access them at any time from your home computer or smartphone. You can also share your forms with your family, friends and caregivers by email, SMS and fax. If you get sick and cannot make medical decisions, having these documents lets others know your wishes about medical care. This ensures you get the treatment and medical care you want. Durante el último año, Inland Empire Health Plan (IEHP) ha trabajado estrechamente con socios de la comunidad para apoyar estos huertos, incluyendo una inspiradora colaboración con Josiah Bruny, CEO de Music Changing Lives (MCL), para traer un nuevo huerto comunitario a San Bernardino.  Situado en la esquina de Electric Avenue y North 40th Street en San Bernardino – lo que una vez fue uno de los basurales más grandes de la ciudad – el terreno se ha transformado en el Huerto Urbano de MCL. El plan médico también patrocinó el “Jardín Comunitario de la Salud”, para ayudar a apoyar un gran huerto acuapónico en la clínica Loma Linda University Health SACHS en San Bernardino.  Este huerto es el primero en su clase en Inland Empire, y cuenta con una gran cantidad de peces que trabajan simbióticamente, nutriendo los canteros y trabajando para crear comida que la comunidad puede cosechar y consumir.  Los huertos están situados en Rancho Cucamonga, Ontario, Jurupa, Perris y San Bernardino. Ultimas noticias - Se presentó un simposio sobre pobreza en la oficina central de IEHP

cuarto Simposio Anual sobre Pobreza de Community Action Partnership of San Bernardino County (CAPSBC) para facilitar conversaciones continuas que beneficien la salud y el bienestar de sus Miembros más vulnerables.  El evento reunió a funcionarios elegidos, legisladores y líderes de la comunidad para compartir soluciones innovadoras que traten los problemas relacionados con la pobreza en el condado. El simposio también incluyó un discurso de David Brady, Profesor de Políticas Públicas de University of California Riverside, y presentaciones de un panel compuesto por expertos de la industria de la vivienda, proveedores de servicios para jóvenes y más.  “Estamos muy agradecidos por la hospitalidad del equipo de IEHP, por habernos dejado presentar el Simposio sobre Pobreza de este año en su centro”, dijo Xiomara Henríquez-Ortega, Gerente de Apoyo Administrativo.   “Community Action Partnership of San Bernardino County valora la asociación que tenemos hace tiempo con IEHP y apreciamos su apoyo continuo para ayudarnos a promover nuestra misión de ayudar a las personas y cambiar vidas. Este evento fue un importante componente de nuestra iniciativa de defensa para influir en el cambio en nuestras comunidades, y es un esfuerzo colectivo en el que estamos trabajando con nuestros colaboradores clave”.  No es la primera vez que se hace un evento positivo y de gran impacto en el edificio Atrium de IEHP, de 400,000 pies cuadrados. Además de ser la base para más de 2,800 empleados de IEHP, el edificio con certificación ENERGY STAR® ha servido a miembros internos del equipo, profesionales de la industria y a la comunidad para celebraciones y en grandes momentos de necesidad.  Los eventos pasados en Atrium han incluido la Mission Conference anual del plan médico, innovadoras exposiciones de tecnología de la información médica, distribución de equipo de protección personal para el COVID-19, súper clínicas de vacunación y más.  “Eventos como el Simposio de CAPSBC sobre Pobreza estimulan conexiones y análisis críticos que hacen avanzar a nuestras comunidades”, dijo César Armendáriz, Director General de Salud Comunitaria de IEHP. “Estamos muy agradecidos por nuestra sólida colaboración con CAPSBC y el excelente trabajo que hacen, y esperamos poder presentar futuras colaboraciones con nuestros socios de la comunidad en nuestro hermoso edificio Atrium, a medida que trabajamos juntos para sanar e inspirar el espíritu humano”.
