Frequently asked questions about CHWs

CHWs are on campus to make getting care easier for you. We partner with your college, so you don’t have to travel or deal with confusing systems alone.

IEHP members can enjoy all CHW support and services at no cost.

No, you can drop by anytime during our campus hours.

Call 1-833-607-0895 and leave a message requesting a call back. Please leave your name, number and the reason for your call.

Your information stays private, so your professors and school staff won’t know anything. We only share it if you say it’s okay or if it’s needed for safety.

Yes. Whether you have questions or just feel stuck and don’t know where to start, we can help you learn about your options and connect you with the right resources.

Yes. A lot of health issues come from things like trouble with food, housing, money or getting around. CHWs can help you find support for all of that because it affects your well‑being.

What to bring to your enrollment appointment

1. Proof of Social Security number (for Covered California, non-DACA recipients only)

Please bring one of the following:

  • Social Security card
  • Tax form(s)
  • Benefit or income statement from Social Security containing your Social Security number

2. Proof of identity

Please bring one of the following:

  • Driver’s license/State-issued ID card
  • School ID with a photograph
  • Social Security card
  • Voter registration card
  • U.S. Military card, draft record, or military dependent’s ID card
  • Government-issued ID with a photograph
  • U.S. or foreign passport (expired passports accepted)
  • Certificate of naturalization or U.S. citizenship
  • Permanent Resident Card (Green Card) or Alien Registration Receipt Card

3. Proof of citizenship or U.S. national status (for Covered California only)

Please bring one of the following:

  • U.S. birth certificate
  • U.S. passport (current or expired)
  • Certificate of U.S. citizenship or naturalization

4. Proof of income

Please bring one of the following:

  • Pay stub
  • Most recently filed Federal Income Tax Form 1040
  • Wage/income tax statement
  • Unemployment/disability benefits
  • Copy of deductions (child support, childcare, alimony, health insurance) (Medi-Cal only)
  • Self-attestation of income in a written statement signed under penalty of perjury may be accepted on a case-by-case basis

Frequently asked questions about enrollment

Yes! We can also connect you to housing, transportation, food, and job assistance through ConnectIE.

It depends on the program:

  • Medi-Cal and CalFresh: Open all year if you qualify.
  • Covered California: Open Enrollment is November 1-January 31. You may qualify for Special Enrollment if you have a life event like losing insurance or moving.

Our trained counselors will check your income and other factors to see which programs you qualify for. They will also help you apply.

Most Medi-Cal members have no-cost plans. If you don’t qualify for Medi-Cal, many Covered California plans are low-cost and sometimes no-cost with financial help.
