Enhanced Care Management

Get the extra care you need

Enhanced Care Management (ECM) is a statewide Medi-Cal benefit, provided at no cost to eligible IEHP members with complex needs to support their health and well-being. This can involve addressing health conditions and challenges, like homelessness, that make it hard to focus on your health.

 

We know that certain health conditions, like diabetes, hypertension, behavioral health concerns or substance use disorder, can be hard to manage. ECM offers extra services to help you get the care you need to stay healthy. These services are at no cost, and you can join or stop at any time.

Who is on your care team?

 

Your care team can support you by phone or in person and may even go to your location. Your care team includes:

  • Nurse care manager
  • Behavioral health care manager
  • Care coordinator
  • Community health worker

What services are covered by ECM?

If you qualify for ECM, a lead care manager will give you personalized support and serve as your main contact. Your lead care manager can meet you at a place that works for you:

  • On the street
  • In a shelter
  • At your doctor’s office
  • At home

 

They’ll coordinate with you, your health care providers and others to ensure you get the care you need. Your lead care manager can also connect you to other community services, such as housing, food and childcare. Your information will remain private and will not be shared without your consent.

 

If you join ECM, your benefits will not change. You keep your providers, and your care team will help you:

  • Find doctors and get appointments for all health needs
  • Improve your health by working with you and your doctor on your care plan
  • Arrange transportation to doctor visits
  • Get follow-up services after you leave the hospital
  • Manage your medicines
  • Connect you to local resources such as food or other social services

Who can get ECM?

ECM is for members, regardless of age, who have Medi-Cal with a managed care health plan and meet at least one of these groups (called “Populations of Focus”):

  • Adults living in a nursing home who want to transition to the community
  • Children and youth enrolled in California Children’s Services (CCS) or CCS Whole Child Model (WCM) with additional needs
  • Children and youth who are or have been involved in child welfare (foster care)
  • Black, American Indian or Alaska Native, or Pacific Islander who is pregnant or has had a baby in the past 12 months

Community Supports

Through ECM, you may also be connected to Community Supports services. IEHP Medi-Cal members have the choice to use Community Supports. If you qualify, there are 14 types of services that can help you with your health and well-being.

 

You don’t have to pay for Community Supports. They don’t replace the benefits that you already get under Medi-Cal. These services are available to Medi-Cal members regardless of whether you qualify for ECM.
