Diabetes Prevention Program

 

 

No cost for eligible members with prediabetes

Prediabetes can develop into Type 2 diabetes if you don’t make some healthy changes. A diagnosis of prediabetes means that your blood sugar is higher than normal, but not high enough to be considered diabetes.

Staying at a healthy weight by eating well and being physically active can reduce your risk of diabetes. Even losing just 5% of your body weight can make a difference. IEHP has a Diabetes Prevention Program (DPP) to support you.

What is DPP?

A yearlong lifestyle-change program to help members with prediabetes make healthier choices to avoid the onset of diabetes. Group classes are taught by a lifestyle coach who will support you to meet your goals. Classes are offered in person and online.

Who can enroll?

  • Members who are 18 years old and over
  • Members who have a prediabetes diagnosis or are at-risk for prediabetes

Diabetic eye exam

Having diabetes puts you at high risk for eye problems that can lead to vision loss or even blindness. That’s why you should get a diabetic eye exam at least once a year, even if your diabetes is well controlled. This exam can help find problems early to protect your eyesight.

 

IEHP members do not need a referral for a diabetic eye exam. Call your eye doctor today to set up this no-cost exam.

New Members: Set Up Your First Visit with Your Doctor

If you’re a new member, please schedule an Initial Health Appointment (IHA) with your new doctor right away, even if you’re not feeling sick. The purpose of your IHA is to help your doctor learn your health care history and needs.

 

During your visit, your doctor may:

  • Ask you questions about your health history.
  • Ask you to complete a health survey.
  • Talk about health education counseling and classes.
  • And more.

It’s a good idea to write out the names of any medicines you take and how often you take them. Make sure to bring it with you on your visit. Be open to talk with your doctor about your health care needs and concerns.

 

All new IEHP members should have an IHA within 120 days of enrolling. Those younger than 18 months need to be seen within two months of joining.

 

Call your doctor’s office today and tell them you want to set up your IHA. The phone number for your doctor’s office is on your Member Card. Be sure to bring your card with you on your visit.
